A. Consent to the general terms

I. I declare,

that I am familiar with and agree to the General Terms and Conditions of Flais Fitness Clubs for the provision of sports services ("General Terms and Conditions") published at the Internet address: https://www.flais.eu/pravila/ with the rights, obligations and the responsibilities of Flais Fitness Clubs and the Client.

For the purpose of granting consent under this declaration "Flais Fitness Clubs” should mean:

B. Personal data processing

II. I declare,

That I have been informed and am aware that my personal data will be processed by Flais Fitness Clubs in the capacity of personal data administrator in accordance with the Company's Policy on the processing and protection of personal data and the documents related to it, available at the internet address:https://www.flais.eu/gdpr/, for the purposes and on the basis of:

• the purchase of subscriptions for the provision of sports services;

• the protection of the legitimate interest of Flais Fitness Clubs for the security and protection of its and my property by performing video surveillance in the fitness centers;

• the fulfillment of legal obligations of Flais Fitness Clubs in accordance with tax and consumer legislation.

III. I declare that I grant my consent:

• a profile for me in the Flais Fitness Clubs customer system to be created;

• my personal data to be processed by Flais Fitness Clubs as personal data administrator for the purposes of creating and administering an electronic profile;

IV. I have been informed about:

1. My rights in regard to:

• Access to my personal data (i.e. to inquire whether my personal data is being processed and to receive a copy of the processed data, as well as information about the purpose of the processing, to whom it has been or will be disclosed);

• Correction of my personal data (i.e. to request correction of an inaccuracy in my personal data or completion of incomplete personal data);

• Deletion of my personal data (i.e. to request the deletion of personal data relating to me in respect of which I have withdrawn my consent to processing);

• Restriction of the processing of my personal data (i.e. to request that the personal data continue to be stored, but not otherwise processed in the event I want the data to be stored for the purpose of defending legal claims);

• Receipt of the personal data, concerning me, in a structured, widely used and machine-readable format (right to data portability);

• Objection to the processing of my personal data for the purposes of direct marketing (i.e. for the purposes of offering goods and services by e-mail, telephone or other direct means, as well as surveying for the purpose of research on goods and services offered), in the event that my personal data is being processed for such a purpose.

2. The right to withdraw consent at any time without prejudice to the lawfulness of processing based on consent before it is withdrawn;

3. The possibility to exercise the above rights through a written or electronic application that meets the requirements of the Personal Data Protection Act, sent to the address of Flais Fitness Clubs in the city of Sofia at 56, Pirin Str. or to the following e-mail address: info@flais.eu

4. The right to file a complaint with a supervisory authority – the Commission for the Protection of Personal Data of the Republic of Bulgaria, in the event that I consider that there is a violation of applicable legislation for the protection of my personal data.
